Adult Membership
Age 13 and older $28/month $139 lifetime activation fee*
Annual Membership Includes:
- Two professional dental cleanings
- Two dental exams
- Any necessary X-rays
- One emergency Exam
- 15% off all other treatments
*$139 lifetime activation fee includes the first month of membership.
This plan is for patients with regular cleanings in the absence of periodontal disease. Some exclusions may apply.
Perio Membership
For patients with periodontal disease
$55/month
$139 lifetime activation fee*
Annual Membership Includes:
- Four perio maintenance cleanings
- Two dental exams
- Any necessary X-rays
- One emergency Exam
- 15% off all other treatments
*$139 lifetime activation fee includes the first month of membership.
This plan is for patients enrolled in active periodontal maintenance due to prior treatment of gum disease. Some exclusions may apply.
Child Membership
Age 12 and younger $24/month $139 lifetime activation fee*
